- Why did you change the name of the company?
- The lease business had operated under a brand license since 1996, when it was spun off with the formation of Lucent Technologies. At that time, Lucent Technologies licensed the prior name for the business to continue to use the name our customers were familiar with. However, after nearly 12 years and subsequent changes in ownership, it now makes sense to change the name. All parties have agreed to not renew the brand license.
- Why did you choose the name QLT Consumer Lease Services?
- QLT is short for Quality. The QLT Consumer Lease Services name has been chosen to reflect the high QUALITY of products, services, benefits and people associated with the business.
- What does this change mean to the lease customers?
- The only change our customers will notice is a new brand name on lease materials and bills. We are continuing to provide the same high quality lease products with convenient services, such as our Unconditional Replacement Policy and Complimentary Next Business Day Delivery Service.*

- Are you still using the same products?
- Yes. A phone is only accepted into our product line if it meets very stringent quality standards. These standards are not changing. The only difference now is the product will be branded QLT Consumer Lease Services."
- Are the lease rates going to change?
- We have no plans to change the rates. We do periodically review the rates as part of our normal business practices. If we should make a decision to change our rates in the future, you will be notified in advance of the change.
- Will there be changes to the Lease Rewards® Program?
- There will be no immediate changes, but we are always looking for new ways to enhance the discounts in the program and will continue to do so.

- How do I exchange the leased telephone?
- With our Standard Next Business Day Delivery Service*, we will deliver the product to your home or office the next business day – at no additional charge. - Does QLT Consumer Lease Services offer products to lease other than telephones?
- Yes. In addition to a large variety of telephones (from basic styles to the 2.4 GHz cordless) we also offer answering systems and specialty products, such as the Call Display 326, and the Auxiliary Amplifier, an amplification device which boosts handset volume up to 4x. - My long cord on my leased telephone is worn out. How can I replace it?
- One of the benefits of leasing from QLT Consumer Lease Services is that you may replace accessories for the leased product – even long cords – at no additional charge! You may also replace cordless batteries and antennas, and answering machine tapes for the leased products. Call our Lease Customer Helpline and we'll make arrangements to send the accessories to your home or office – at no additional charge.
- What if I move? Can I take the leased products with me?
- Yes. You can even exchange the current leased product for one that matches
your new décor! The fact is you can move leased products to any new home or
apartment – anywhere in the 50 United States. Just be sure to notify us of your
address change by calling our Lease Customer

- May I buy the leased telephone?
- Leased telephones are not for sale. When you lease a phone, you are covered
by the Lease Service Benefits, which include convenient same or similar model
replacement of the leased product for any reason. You may also exchange the
phone for a different color or style. With our Flexible Lease Options you can
exchange the leased phone, anytime, for any reason. Leasing provides our
customers with peace of mind from worrying about replacing or exchanging

- How do I return a leased telephone?
- If you decide to end your lease, we can send you a postage paid return mailer which can be dropped off at your local post office. Call our Lease Customer Helpline. Please consider what you will be losing if you decide to return your leased equipment. You will be responsible for paying for any repair or replacement costs for your own phone, should the need arise, and you will be missing out on the great savings associated with the Lease Rewards® Program – up to a 60% discount on prescriptions drugs, a 20% to 60% discount on vision care, a 25% to 50% savings on vitamins, and 10% to 20% savings on hearing care!
- What is the Lease Rewards® Program?
- The Lease Rewards® Program gives you access to exclusive savings for you and your family to enjoy. The Lease Rewards® Program includes vision and hearing care product discounts along with prescription drug discounts, discounts on vitamins, and special coupons from national retailers and merchants. You will receive a Welcome Package in the mail with a description of your benefits, as well as the FREE Lease Rewards® card. Present the card at any of the participating vision or hearing centers or pharmacies to receive your discount. The discounts on the vitamins are available through mail order. - What can I save with the Lease Rewards® Program?
- You can save 20% to 60% on frames and lenses, including contact lenses, at participating vision care product centers nationwide. You can save 10% to 20% on hearing aids and hearing care products and services. You can save up to 20% on name brand and up to 60% on generic prescription purchases at over 55,000 participating local pharmacies, online at the on-line drug store or through the mail. You can save 25% to 50% on vitamins and supplements through the Healthcare Direct Mail Order catalog.
- How much does the Lease Rewards® Card cost?
- The Lease Rewards® Card costs QLT Consumer Lease customers absolutely nothing. You and your immediate family including your spouse and children up to the age of 23 who are still living at home can enjoy all of the savings the card has to offer for as long as you remain a valued lease customer.
- How do I get my discounts on prescription drugs?
- To get your prescription drug discount, simply show the Lease Rewards® Program Card at a participating pharmacy and you can receive up to 20% off brand name prescription medication and up to 60% off generic drugs. Participating pharmacies are nationwide and include chains such as Eckerds, Rite-Aid, K-Mart and more! Locate a participating pharmacy in your area by using the Provider Search.
- How do I get my discounts on vision or hearing care products?

The first thing you will need to do is request your card. When you pay for your vision or hearing care products, or prescription drugs present the Lease Rewards® Program Card and save 20% to 60% on vision care and 10% to 20% on hearing care products. Your welcome kit will provide a list of a few of the closest vision care and pharmacy providers in your area or if you prefer, you can click Provider Search, and put in your Zip Code to obtain the closest locations. Hearing care locations can be obtained by calling the Lease Rewards Center, toll-free on 1 (888) 831-4311. The vitamins and supplements discount is offered through a mail order catalog. You can request a catalog by calling the center, also.
- Does this program cover my entire family?
- All members of your immediate family including your spouse and children up to the age of 23 who are still living at home may take advantage of the Lease Rewards® Program benefits.
